Oak Vale Gardens is an 18 bedded Specialist Neuro-rehabilitation Unit in Liverpool.  We are part of the Cheshire and Merseyside Rehabilitation Network and work closely with the Walton Centre NHS Foundation Trust.  We work as members of an interdisciplinary team, including physios, OTs, nurses and Psychologists, providing nursing care and neuro-therapy to patients with traumatic brain injuries or complex neurological conditions.

As a critical member of the multidisciplinary team, you will be supporting and assisting Registered Nurses in the assessment, planning and implementation of patient’ care, delivering care in line with the support plans and guidelines as well as  liaising with family members as appropriate.

Responsibilities:

 

• Assists qualified staff with the assessment and implementation of individual patient care plans undertaking routine tasks and activities as directed to facilitate the wellbeing, dignity and treatment of all patients.

 

• Provides practical support and responsive care to patients who require assistance with  personal needs such as dressing, bathing and toileting activities.

 

• Observes and monitors the wellbeing of patients, ensuring that any unusual physical, mental or emotional occurrences are promptly reported to Nurse in Charge, and documented as appropriate.

 

• Completes mandatory and bespoke training.

 

• Contributes to a team approach to patient’ care in conjunction with all members of the multidisciplinary team.

 

• Assists in the maintenance of ward cleanliness and tidiness and adhere to infection control policy and procedures at all times.

 

• Develops and maintains a good professional relationship with patients, colleagues and visitors responding promptly and courteously to requests and enquiries.
