Job Advert Role Description

Kemple View is a 90 bedded hospital split across two service lines – secure and rehabilitation and recovery. We cater to the treatment needs of male patients with a mental illness and/or personality disorder. We aim to maximise an individual's opportunity for recovery, to lead a more independent life back in the community wherever possible.

We have six wards including Low Secure and Locked Rehab for male service users only.

Many of our patients have previously been in high, medium or low security. We aim to work collaboratively with our patients enabling them to take increasing responsibility for managing their own risks leading to a controlled and graded discharge. Where a patient requires greatest support we offer contextual risk management, enhancing compliance and aim to improve their quality of life. We serve the patient, carers and visitors.

Kemple View also has a dedicated and purpose-built ward offering Low Secure Forensic Specialist Older Age service for patients over the age of 50.

A higher profile is given to this patient group's physical healthcare, risk reduction and where required, mobility and daily living aids.

The Integrated Practitioner is a key clinical role and will be based across the 2 x R&R wards supporting both social work and nursing duties. This is a new role and you will be supported to develop and balance the competing demands of the two professional domains. You will be willing to take on additional training (eg. Safeguarding Officer) and will work with the nursing team and the MDT to promote excellent standards of patient care at all times. 

Main Duties:


Nursing:
• Act as the Nurse in charge of the Ward.
• Supervises a defined group of Nursing or HCA colleagues, ensuring they receive management and clinical supervisions 
• Prioritise nursing duties and delegate appropriately and effectively within the shift co-ordination Framework, working as nurse in charge as required, ensuring as far as reasonable practicable a safe environment. 
• Recognising and responding appropriately to challenging behaviour in line with company and divisional policies and training; supporting junior staff in managing difficult situations and de-escalation, and developing resilience and strategies to manage.
• Monitoring, evaluating, and reporting on patient care outcomes as well as working with complex patients and providing support to MDTs. 
• Adhere to Company Medicines Management policies at all times. 
• Adhere to Priory Group policies and protocols and the NMC Code
• Maintain a continued level of personal professional development to demonstrate contemporary practice and meet the requirements of revalidation, as a minimum


Social Work:
• Providing accurate information about care in an accessible format to patients/service users and their families
• To ensure an accurate, holistic and up to date social history is available for each patient, which offers formulations and recommendations.
• With the patient’s permission initiate visits, maintain contact with patient’s families and/or significant others offering support, information, and where relevant to the care plan joint family and patient sessions.
• Where it is part of an agreed care plan, to carry out focussed casework with individual patients and their families using a range of interventions under supervision.
• To lead and co-ordinate the process of section 117 Mental Health Act aftercare plans with the MDT and relevant community agencies, which meet the needs of the patient, and takes full account of risk factors.
• To be aware of and adhere to responsibilities for people under CPA, including offering/arranging Family and friends, carers support.
• To liaise professionally and sensitively with other involved parties in order to provide seamless and consistent high quality care for service users under care of the team. This includes, but is not limited to; carers, families, internal and external colleagues, and voluntary services as appropriate 
• Ensure awareness and knowledge of the local child and adult safeguarding policies and procedures in the service, in addition as to whom to contact with the Local Authority Safeguarding team;
