Job Advert Location Description

Bannister Farm supports adults with a primary diagnosis of autism and learning disabilities. Who may require support with some mental health requirements. We will provide a person centred approach, ensuring individuals have a care support plan and varied activity program based on the individual’s choice and preference. Individuals are supported using positive behavioural support, which may include physical intervention strategies and a PBS practitioner supports the service. Improving life skills and independence will be an area of focus and communication of such activities will be discussed with the speech and language therapist and occupational therapist. Each person at Bannister Farm will have an individualised communication plan, which includes strategies in place in order seek their wishes and feelings. These may include a range of alternative means of communicating e.g. use of electronic communication devices, symbols, photographs and objects of reference, which the person may use to express themselves. In instances of where this is not always apparent of difficult to ascertain, their key-worker would be asked to give their opinion based on their knowledge of the person’s body language, behaviours and vocalisations as well as working alongside family and others who know the people well to ensure their wishes are known.

 

Bannister Farm is divided in to 3 areas. The main house has 3 bedrooms 2 upstairs and 1 down. All bedrooms have en-suite facilities. There is the main kitchen/ dining area, which all the main house residents share. The residents have support from staff to prepare all their meals. There is also a large shared lounge with an adjoining conservatory.

 

Annex 1 is a small one bed flat with en-suite facilities and its own kitchen/ lounge.

 

Annex 2 is a larger flat it consists of lounge an ADL kitchen to prepare meals and carry out independent living tasks. It has one bedroom and a large shower room.

 

The grounds are quite large consisting of a large rear garden with a summerhouse for all the residents to use individually. The main house has a small courtyard directly off the kitchen and dining area.

There is a large tarmacked area to the front of the property and the entire property is fenced off.

Job Advert Role Description

Prepares plans and delivers a wide range of activities and programmes that are tailored to meet the needs and abilities of the individual, whilst also providing group activities and programmes that will bring the individuals together.

 

•          Participates in site wide programmes and activities, and in other home programmes, with the agreement of line manager.

 

•          Observes, records and reports Service user involvement in activities.

 

•          Ensures the effective, timely and accurate updating of healthcare and other records and computerised information systems as directed.

 

•          Is able to creatively plan activities to meet the needs of service users.

 

•          To liaise with a wide range of external educational, vocational and leisure activity providers to widen the range of opportunities available to the people that we support.

 

The role holder should present the service in a positive light and should be proactive in making suggestions for how to improve the service.
