Job Advert Role Description

Based in the historic city of York, Stockton Hall Hospital is a medium secure 112 bedded unit spanning over 7 wards, caring for people with mental illness, personality disorders and learning disabilities.
As a Registered Social Worker, you will deliver high quality, safe care, ensuring consistently high standards of practice, which comply with Social Work England Code of Practice. The post holder will co-ordinate, negotiate and manage the mandatory processes and procedures to ensure the unit’s compliance with current legislation and statutory regulation. Taking appropriate action to maintain a humanistic and compassionate ethos whilst working with complex situations of social need and risk, including balancing competing views and rights.
The role includes working with the Multi-disciplinary team to deliver person centred care in a collaborative way focusing on personalisation and recovery and supporting individuals to make positive, self-directed change.
Other responsibilities include but are not limited to:
- Identify any social issues which a patient may be experiencing and ensure these are addressed within risk assessments and care plans and make sure the patient is supported.
- Create links between the care team on the ward and the patient’s community team, ensuring that early discharge planning is arranged and community teams are fully up to date and aware of the care pathway of the patients.
- Develop links between different agencies within a patient’s local community, for example Care Co-ordinators, Children’s Services, families/carers and future possible placements.
- Take a proactive approach in supporting the ward within the hospital with monitoring any safeguarding concerns amongst the patient group on the ward, ensuring the safety of patients at all times.
- Complete investigations into incidents in the hospital and feedback this to the Senior Management Team.
- Investigate complaints made by patients in the hospital as required by the Complaints and Compliance Administrator.
- Support carers and family members of patients, signposting them towards appropriate links within their local community to ensure they receive the appropriate support.
- As the senior member of the department, you will actively engage within the Clinical Governance of the hospital, attending meetings, providing feedback to the Hospital Director and working towards the agreed goals and objectives of the hospital.
- Acting as the main contact between relatives, agencies and other official organizations taking a lead in negotiations and communications regarding patients, coordinating multi agency liaison.
- Looks at potential safeguarding risks around young people’s contacts, including placing management measures around this such as supervision by staff.
- You will look at the potential risks around home leave, this includes environmental and emotional.
Experience and qualifications required:
- Degree level Social Work qualification BSc or MA
- Registration with Social work England
- Detailed knowledge and understanding of current legislation
- 2 years’ experience within a similar setting (forensic, criminal justice or mental health, children’s Services) including completion of ASYE.
