Hazelwood House is a low secure unit for adult males with learning difficulties and mental health needs based near Chesterfield and we were recently rated as outstanding by the CQC.
We are looking to recruit a dynamic individual to join our Occupational Therapy Team as an Occupational Therapy Assistant. The ideal candidate will be able to demonstrate a positive, creative and an imaginative approach to problem solving and delivering therapeutic activities in a Rehabilitation and Recovery setting.
You will work with the Occupational Therapist in producing an Activities Programme for patients.
As part of a wider Multi-disciplinary team the Occupational Therapy Assistant will assist our Occupational Therapist, Healthcare Assistants and Nurses in the assessment, planning, implementation and evaluation of individual care plans.
Alongside the Occupational Therapist, the ideal candidate will respond to referrals made by the therapy team supporting the completion of risk-assessments and support plans to best meet the needs of individual patients.
The successful candidate will have excellent listening, communication and report writing skills. You will be motivated, highly reliable and able to work within an emotionally demanding environment.
You will collaborate with professional and clinical colleagues, participating in departmental meetings and occasionally contributing at case conferences or multi-disciplinary team meetings as directed
The successful candidate will provide support and facilitate individual and group sessions to our patients, working collaboratively with individuals to promote their recovery and enhance their social inclusion
You will need to maintain accurate and up-to-date records ensuring important information, observations and events are recorded and/or reported as appropriate in accordance with Priory policies and procedures
It is desirable that you will have relevant experience within mental health services.
